This form will notify us that you want to arrange pickup and or delivery of your equipment for repairs. We will be contacting you to alert you when the truck will be picking up or delivering your equipment.

We will send you a copy of this request for your records to the Owner Email address.
Please tell us which units you want to be serviced and any details about any issues or repairs you want to be done to each. If we have questions, we will contact you. Their is space for up to three.

This service is charged for by the trip and distance from the repair center. We compute distance by shortest distance via google maps. The rate per mile is $2.00 each, with a minimum charge of $20.00 per trip. We can pick up up to three pieces of equipment in one trip.
